LPN/LVN / Care Coordinator
LPN / Care Coordinator Kelly Services has a unique opportunity available for an LPN / Care Coordinator to join our client located in Wilkes-Barre, PA.

Responsibilities:
This position is focused on improvement of HEDIS measures.
You will be accountable for:
Review of member data in clinical workflow system and excel files related to condition - specific and preventative gaps in care Conduct telephonic outreach to members with gaps in care to educate on the importance of adherence, encourage completion of screenings, and support coordination of services between members, physician offices and ancillary services for the purpose of closing the screening gaps.
Accurate and timely documentation of calls in Excel and the clinical workflow system REQUIRED
Qualifications:
Active LPN license in the state of Pennsylvania Computer literate Associates Degree or higher required Flexibility to work a 10a-7p or 11a-8p schedule HOW TO APPLY:
