Registered Nurse-High Risk Nurse Case Manager
Duties include but not limited to the following:
Maintains oversight for the High Risk Case Management Program. Provides case management/monitoring services to veterans with suspected or actual high risk diagnoses through establishment of a final diagnosis and/or treatment. Facilitates effective patient navigation within a complex health care system to decrease fragmentation of care and coordinate services. Ensures coordination of interdisciplinary treatment team interactions, keeping team aware of identified patient care issues or concerns. Maintains a tracking system to monitor patient progress through the plan of care. Functions as a resource and provides consultation to members of the interdisciplinary team and other clinical staff in support of implementation of case management initiatives. Collaborates with the Chief and Associate Chief of Medical Service, in identifying opportunities for improvement and assisting with problem resolution to ensure optimal and appropriate patient care delivery and cost effective use of resources. Provides support for case management activities as identified by the Chief of Medical Service, Associate Chief of Medical Service, and Chief of Oncology. Maintains knowledge of case management principles including case management models, tools, processes, goal recording & documentation, quality improvement, and performance improvement measures. Maintains knowledge of case management principles and strategies including confidentiality, conflict resolution, and communication and negotiation skills. A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The appointing official may waive this registration if the RN is to serve in a country other than the U.S. and the RN has registration in that country (e.g., Philippines). The RN must maintain a current, full, active and unrestricted registration to continue employment with VA The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. In these programs, students are given a certificate of professional nursing to sit for the NCLEX and should submit this certification to VA prior to appointment. Students do not earn a BSN, but receive a MSN degree upon completion of course work. A copy of the MSN transcript must be provided to VA. Bridge programs that confer a master's degree also fully meet the education requirement, even though a Bachelor's Degree is not awarded. Case Management experience preferred. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
