Lead Medical Instrument Technician - Cardiac Catherization

The Lead Medical Instrument Technician in the Cardiac Catheterization Lab is responsible for:
-ordering Cath Lab specific supplies including catheters, balloons, stents, sheaths, etc. -coordinate communication with vendors and schedule visits. -scrub, circulate, monitor, and operate equipment on the most complex examination and treatment procedures including intra-aortic balloon pump therapy. -assure medications are properly labeled and available for physician administration. -knowledge of cardiac medications use, appropriate doses, and side effects and assist with training and mentoring other technicians and staff -assist the cardiology manager with quality control and performance improvement activities. -work with physician to complete both diagnostic and interventional invasive cardiac, peripheral and electrophysiology procedures including:
LHC, RHC, PTCAs, PTAs, stent placement, IABP, IVUS, pressure wires, pericardiocentesis, temporary pacemakers, radial artery procedures, peripheral procedures, permanent pacemaker placement and AICD placement. -monitor progression of procedures requiring independent judgment and anticipate need to provide specific instrument and/or catheters. -intimate knowledge of vascular and coronary anatomy and physiology related to cardiovascular function. -adhere to all departmental, service, hospital, agency, and national regulations and policies and must be knowledgeable in Picture Archiving & Communication Systems (PACS) and specialized equipment employed for digital subtraction systems and interventional procedures. -actively participate in departmental and facility wide committees such as Radiation and/or Laser Safety. Essential functions and responsibilities include:
Ability to scrub, circulate, and monitor advanced procedures (including electrophysiological) to include intravascular ultrasound, RADI or wave-wire, Angiojet, filter wires, permanent pacemaker insertions and AICD insertions. Prepare contrast materials under the direction of the physician for the purpose of performing exams requiring injectable solutions. Must be continuously on the alert to the patient's condition and other disorders. Adverse reactions on the part of the patient require immediate attention so that harm does not come to the patient or equipment. Display advanced knowledge of equipment, medications, and supplies used and the ability to set-up, calibrate, operate, and troubleshoot the equipment. Understanding of waveforms from the great vessels and all chambers of the heart including the ability to recognize and distinguish normal from abnormal, artifact from abnormality and forewarn physician. Displays knowledge of quality control and performance improvement indicators for the Cardiac Cath Lab and ability to prepare and analyze quality control test results. Performs daily inspections of radiographic units to ensure proper mechanical functionality. Perform monthly, quarterly, semi-annual and annual QC testing when indicated. Coordinates all scheduling of preventative maintenance for modalities. Maintains accurate records for quality assurance, quality control, performance improvement and other related purposes. Check expiration dates of lab stock routinely and ensure adequate amounts of supplies are kept at par level and review consignments or contracts to ensure they are kept current and funded. Display knowledge of more complex examination/treatment procedure and techniques in order to provide training to aid in the evaluation of performance improvement projects, and to assist in writing policies, procedures, and protocols that pertain to the Cardiac Cath Lab. Display advanced knowledge of complex and non-standard treatment and examination procedures and techniques. Knowledgeable of radiation protections standards, minimum exposure techniques, and appropriate beam limitation. Knowledgeable of the technical adequacy of the digital image, including the ability to adjust the image quality in the digital system. Knowledge of anatomy and physiology and cross-sectional anatomy, recognizing unusual images, and determining proper positioning. Practices radiation safety. Knowledge of patient's clinical record, diagnosis, and laboratory result and demonstrate appropriate techniques to gather relevant information from medical record, significant others, and health care providers. Exercises skill in using tact, diplomacy, and courtesy is dealing with the customer base, patients' staff, family, visitors, and volunteers. Ability to communicate effectively and professionally with employees at varying grade levels. Demonstrates skill in problem solving and conflict resolution. Keepsabreast of technological advances in the fields of Cardiac Cath/Peripheral Vascular and Electrophysiology and incorporates these advances into practice. Other duties as assigned. Work Schedule:

Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. For certain functional areas at the higher levels or supervisory assignments, specific certifications appropriate to the specialty are indicated in this standard under Titles and Certification/Registrations. Grandfathering Provision. The following is the standard grandfathering policy for all Title 38 Hybrid qualification standards. Some of these provisions may not apply to this occupation. Please carefully review the qualification standard to determine the specific education and/or licensure/certification/ registration requirements that apply to this occupation. All persons employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:
Such employees in an occupation that does not require a licensure/certification/registration, may be reassigned, promoted, or demoted within the occupation. Such employees in an occupation that requires a licensure/certification/registration, may be reassigned, promoted up to and including the full performance (journey) level, or demoted within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions. Such employees in an occupation that requires a licensure/certification/registration only at higher grade levels must meet the licensure/certification/registration requirement before they can be promoted to those higher grade levels. Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation. If an employee who was retained in an occupation listed in 38 U.S.C. 7401(3) under this provision leaves that occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. Cardiac Catheterization Technician GS-9 Experience. At least 1 year of experience comparable to the next lower grade level, which is directly related to the duties of the position to be filled, and must fully meet the KSAs at that level. This would be experience which demonstrates possession of the knowledge required in order to provide all aspects of cardiac catheterization technician services. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk ( ):
1. Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols, that pertain to the Cardiac Catheterization Laboratory. 2. Knowledge of JCAHO and other regulatory requirements. 3. Ability to communicate effectively and professionally with employees at varying grade levels. 4. Ability to manage and supervise employees. 5. Ability to provide, or provide for, staff development and training. Certification. Registration by Cardiovascular Credentialing International (CCI) as a registered Cardiovascular Invasive Specialist (RCIS) is highly desirable. Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications are also desirable for employees at this level. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and range of variety, and be performed by the incumbent at least 25% of the time.
